Welcome to the new electronic Distribution Center Sale Items Order Form. The Iowa Department of Transportation has developed an easy way for you to fill out this form on-line.

COMPLETION INSTRUCTIONS

After opening the PDF form, you can click the Highlight Fields icon in the upper right to have the form’s blank entry fields appear in light blue. After filling in your customer information at the top, click on the first field in the Qty. column. If you want to order this item, simply enter the number you desire, then press your ‘Tab’ key. Notice the Total of that item is automatically calculated for you. Continue through the form entering the quantity you desire for each item in the Qty. column. The total of all items are also calculated for you.

Required Fields (red outline when highlighted): Most of the fields appearing at the top of the form must be completed in order for you to print or save the form.

Recommended Fields: If you have an email address, please enter it in the email field to help us expedite your order. You may leave this field blank if you do not have an email address. If your mailing address includes a post office box, please enter the number in the P.O. Box field.

Shipping Fees: A shipping and handling charge of $7.00 will be added to all orders.

Sales Tax: All non-governmental agencies or individuals within the State of Iowa are required to pay county sales tax. The sales tax is also calculated for you based on the sales percent established for your county. Out-of-state agencies or individuals are not required to pay sales tax.

PRINTING INSTRUCTIONS

Click the Print Form button in the upper right corner of your form to print a hard copy of it.

MAILING INSTRUCTIONS

You will find the total amount due for your order in the Total Payable to Iowa DOT field.
