Multiple routes may be included on this form if the exemption justification is the same for all of the submitted routes. 

If multiple routes are being submitted, please enter “Multiple routes” in the Route field and “See attached map” in the From and To fields.  In this instance, a PDF map must be attached in the Justifications section below. 

If routes have differing exemption justification reasons, they must be submitted separately.

Beginning July 1, 2025, the Department may issue all-systems overweight permits on all secondary roads, except those roads for which an all-systems overweight permit is not valid.  In order to designate a route as not valid, the county must submit a written Justification Report to the Department.
